Job Title: Link Worker – Social Prescribing

Location: Cromer, North Norfolk

Salary: £22,462 to £25,295 per annum

Job type: Full time (37 hours per week), Fixed Term (2 Years)

North Norfolk is an attractive rural District with a beautiful coastline, fantastic landscapes and big skies within easy reach of the fine City of Norwich, however, some residents need additional help and support to maintain and improve their holistic health and wellbeing. This post offers an exciting opportunity for the successful candidate to deliver a Social Prescribing service which will improve peoples’ lives

Main Responsibilities:

As the Link Worker – Social Prescribing, you will work with residents who need help and support to manage issues in their lives which are having a negative impact on their general health and wellbeing. The Council’s aim is to reduce or prevent the number of unnecessary consultations with General Practitioners and prevent or delay the need for ongoing health and social care services.

Key Requirements:

  • To develop the Link Worker Social Prescribing service through working collaboratively with the PCNs, organisations in the North Norfolk Help Hub and other statutory and voluntary services
  • To have a regular and visible presence, directly link and engage with the multi-disciplinary teams in each practice in NN1 and NN2
  • To establish contact with those referred in to the service by phone, face to face in practices, community locations or in people’s own homes to identify the issues which are impacting on their lives and to help them manage issues presenting in their daily life
  • To promote and raise awareness of local opportunities to forge strong links with local voluntary and community sector providers of health and wellbeing activities
  • To identify gaps in local services and community resources and support voluntary sector organisations to develop new community based programmes and activities which meet identified needs
  • To collect and manage information and data to support management of referrals, case management, monitoring and project evaluation
  • To effectively manage cases, assess and input information and data onto identified systems, establish system of regular feedback to individual practices
  • To act on all safeguarding concerns in relation to those being referred to the service
