Job Role:
We are looking for a part-time Floating Support Worker to support those people who are 25+ and at risk of becoming homeless, within the borough of Solihull, working 30 hours per week.

This is a great time to join SJMT at a time of growth and development in the charity, we also provide Extra Care and Residential Care services in Solihull and we have housing schemes for older people in Solihull, Shirley, Wolverhampton, Erdington and Kidderminster.
Key responsibilities:
- To provide key worker support to a specified number of service users, including one to one individual support and liaison with other agencies on behalf of people who use our services
- Enabling individuals to self-manage their personal and domestic resources and to undertake activities of daily living
- To carry out all needs assessments, individual safety and support planning within organisational and legislative requirements.
- Liaising with other external agencies and community groups on behalf of people who use our services
- To undertake assessments in conjunction with statutory partners as appropriate to identify housing related support needs of those referred to the service
- Contribute to the protection of individuals from the risk of abuse and harm To monitor and report to senior staff on the effectiveness of support delivery
- Encouraging service users to recognise, understand and manage factors that affect their mental wellbeing and physical health
- Participate in the monitoring requirements of the service by providing statistical and qualitative information on activities, success stories and outcomes achieved
- Ensure effective incident and complaint recording including using the relevant SJMT systems and process
General Requirements:
- Caring and enthusiastic
- Experience of providing support to people who are homeless or at risk of homelessness; or experience of providing support services within another social care setting in either a paid or voluntary capacity.
- The ability to make a comprehensive assessment of an individual’s situation and needs;
- Equivalent work experience and lived experience will be considered
- Great organisation skills, listening skills, good written, non-verbal and verbal communication skills
- The ability to work as part of a team
- Must understand and share the Values of the Organisation.

- Full driving licence and use of car
- Two Covid-19 vaccinations
- A DBS check will be taken up for the successful candidate
