About the Role:

This role is to ensure that our residents are gaining the best domiciliary care and individuals are living the ‘good life’. Working in a stunning environment at Morden College you will bring fun, energy, and lots of laughter. You will oversee individual’s personal plans, implement initiatives to support their needs, embed schemes to promote community engagement and so much more!

Job Summary/Purpose:

  • To lead a Domiciliary Team that support people to live the “good life” in independence by promoting dignity, independence, choice, and fulfilment within a policy of managed risk taking.
  • Ensure that all staff and volunteers adopt and maintain a person-centred approach to the development and delivery of high-quality services and uphold the principle that each individual is at the centre of our services and the support they require.
  • Work with individuals to make sure that their personal plans promote wellbeing and enable them to be as independent as possible, managing risks by thinking creatively about options for safe solutions.
  • Implement an efficient and effective domiciliary care coordination system for people living in independence to ensure their individual needs are met through timely interventions, by care workers who have the right skills, knowledge, and attitude to enable individuals to live in independence for as long as is possible.

You will be yourself, have a sense of fun and take ownership of all aspects of your role, whilst being a positive role model for staff and an ambassador for Morden College.

Key Duties & Responsibilities:

  • Ensure that an individual’s personal plan promotes wellbeing and enables them to be as independent as possible, and that risks are managed by thinking creatively about options for safe solutions that enable individuals to do things that matter to them.
  • In partnership with colleagues identify those people who are at risk of loneliness and social isolation, reduce and maintain a reduction in the risk of social isolation by implementing initiatives to facilitate, and support that aligns to the individual’s needs, aspirations, and concerns.
  • Work together with the Health and Wellbeing Team and Clover Primary Care Service to ensure that where there are changes in a person’s health, that impacts on their wellbeing and or care and support needs this information is shared appropriately, and action taken to ensure timely interventions that enable the individual to continue to live safely in independence for as long as is possible
  • Work in partnership colleagues to embed projects such as Compassionate Neighbours across all schemes to promote community engagement, aimed at helping beneficiaries to build connections with the wider community and to improve their physical and emotional wellbeing.
