The Role:
Due to continued growth, we are looking for a Case Manager with a minimum of two years case management experience to join our team, providing clinical oversight to children and young people with a range of needs (traumatic brain injury, acquired brain injury, spinal cord injury, amputation, and poly trauma).
The Candidate:
We are seeking an individual who is highly competent and dynamic in developing client centred rehabilitation packages with knowledge of working within a variety of funding streams. Our Case Managers work primarily from home, with some occasional attendance at our office, along with regular face to face contact with clients and their wider team.
Responsibilities:
The Associate is responsible for undertaking the required duties of a Clinical Case Manager which includes (but is not limited to):
- Provision of pro-active clinical case management services to the Client;
- Developing rapport and an appropriate therapeutic relationship with the client and their family;
- Provision of education, emotional support and guidance to the Client and their family;
- Clinical management and coordination of rehabilitation and care and support packages (some may include regulated activity – personal care). This may include HR and management of teams in the context of employment law;
- Creating an effective and appropriate communication strategy with the client and all relevant stakeholders;
- Regular coordination and communication with instructing parties, Clients, families, services, professionals, agencies, NHS, Local Authority and all relevant stakeholders;
- Conducting ongoing assessments, outcome measures and monitoring of the Client’s situation, reviewing relevant documents and making adjustments to the services offered;
- Production of Immediate Needs Assessment Reports (meeting with Clients, completing assessments, research, travel and writing reports) as required and in line with the Company’s INA Service Agreement;
- Production of quarterly progress reports for Clients with active litigation in line with our Case Management Service Agreement;
- Advocacy, managing expectations, fostering independence, supporting decisions, developing case management plans and goal setting as required and in preparation for quarterly progress reports;
- Managing the provision of specialist care, adaptions, accommodation, treatment, vehicles, augmentative communication devices, wheelchairs and specialist equipment as required;
- Assessing and managing risk, safeguarding, capacity or any other professional activities as set out in the relevant Company policies;
- Adhering to relevant regulations and professional guidelines as set out by CQC, BABICM, IRCM, MTSP, HCPC, SWE, NMC, RCOT, CSP etc
Desirable Requirements:
- Advanced BABICM Member. – If obtained, the pay rate would start from £56 per hour.
