Job Title: Carer’s Service Link Worker

Location: Manchester, M15 5FS

Salary: £21,748 p.a. pro rata

Period of Contract: FIXED TERM- March 2023 – 21 Hours per week (requirement of flexibility for the needs of the service)

About Us:

ACCG are working in partnership with LMCP and ACMHS plus other associates to deliver carers services in the Central Manchester Locality. Our aim as a partnership is to promote the Carers Pathway within our local communities and to identify unpaid carer’s from BAME backgrounds early on in their caring role and support them to access carer’s services.

The Role:

We are delighted to offer an exciting opportunity to join the ACCG and partner organisations in the role of Carer’s Service Link Worker. You will be required to work in a team to identify unpaid carers in Central Manchester and support them to access available Carer’s services. You will develop and maintain relationships with other VCS organisations within Central Manchester, as well as individuals and statutory services that support Carer’s. You will have experience in supporting those from underrepresented groups, in particularly those from an African Caribbean or South Asian background.

Main Role and Responsibilities:

  • Identify, develop and maintain relationships with professionals/organisations/groups, VCS organisations & individuals in Central Manchester that may support carers, with an emphasis on those that work with excluded groups.
  • Raise awareness of these individuals and VCS organisations of the range of services available through Carers Manchester Pathway and Contact Point.
  • Support individuals and Voluntary, Community and statutory (VCS) organisations to develop links with Carers Manchester Contact Point (CMCP) and carers service providers.
  • Support Carers to be socially connected and not isolated.
  • Support individuals and VCS organisations to identify gaps in provision and work in partnership with others to develop appropriate support and services for their communities/members/service users
  • In partnership with individuals and VCS organisations identify unmet needs and feedback through the Management Team and other appropriate channels.
  • Identify and promote volunteering opportunities to individuals and VCS organisations.
  • Work as a member of a team.
  • Promote the project to colleagues through specialist knowledge and experience.
  • Maintain up-to-date knowledge of current Health and Social Care policies, legislation and practice
  • Maintain accurate, up to date records of carers in touch with the service, using the ACCG’s database and other necessary monitoring processes
  • Support the delivery of the Central Manchester Carers Locality Partnership (CMCLP) communication plan
  • Be the link between each of the Partner Organisations and the CMCLP.

Other Duties and Responsibilities:

  • Complete specific tasks allocated through work plans, project plans and the business plan.
  • Provide monthly information (accurate data and informative commentary) within your areas of responsibility for performance management purposes.
  • Be involved in producing information for annual impact reports.
  • Actively engage in ongoing personal and professional development, making full use of supervision, appraisal and learning opportunities.
  • Promote equality and diversity at all times ensuring inclusive and integrate services.
  • Seek advice, support and guidance as required.
  • Maintain a general understanding of the work of the whole organisation and attend team meetings/events as appropriate.
  • Adopt a flexible approach to working patterns to suit the needs of the role and responsibilities as required.

  • Communicate effectively with carers
  • Recognise people’s needs for any preferred alternative methods of communication and respond accordingly.

  • In the course of seeking support, carers entrust us with personal and sensitive information including their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately under the data protection rules.
  • The post-holder will have access to confidential information relating to service users and their careers. All such information from any source is to be regarded as strictly confidential. The post holder will maintain and promote confidentiality of information for carers, service users and businesses in accordance with the Data Protection Act.
