What does a Social Worker do at Hillingdon?
Hillingdon Adult Social Care has three locality teams working with adults with a physical disability and older peoples. The team responds to safeguarding enquiries, carries out best interest assessments for service users, implementing and reviewing care packages in the community (Service users’ homes / Supported Living / Care Homes).
The successful applicant will work closely with the CCG, GP’s, Community Health teams and voluntary sector agencies. We are looking for an enthusiastic social worker with skills and knowledge in the Adult Social care field, focusing on person centred and strength-based approach in social care need assessment, support planning, mental capacity assessment and best interest decision making, S42 enquiry and DoLS challenges.
Job Requirements:
You will be a qualified, experienced Social Worker looking to progress your career or an accomplished Senior Social Worker, with a wide range of experience working with Older Adults and/or People with Disabilities. You will have developed your skills and knowledge with further post qualifying training and have specific experience with the relevant client group.
You will be innovative and creative with a willingness to explore new models of working to improve the outcomes for our service users. Moreover, you will be a highly motivated individual who is able to build strong relationships to deliver the best possible outcomes for vulnerable people.
