Hours and Shifts Available: The position is 37 hours per week on average, consisting of a mixture of working with ‘Adam’ and administration time
Shifts vary depending on school holidays
Current shifts are:
Term Times: 16:30-20:30 or 20.00-08.30 weekdays; 09.30-20.30 weekends
School Holidays: 10.00 – 14.30 or 14.30 – 20.30 weekdays; 09.30-20.30 weekends; 20.00-08.30 nights
The Team Leader will be expected to also work at times outside of these hours to provide training, supervision and support to the team of staff and ‘Adam’.

The Role:
‘Adam’ is 16 years old and sustained an injury soon after birth, as a result he has a diagnosis of cerebral palsy which affects his communication, cognition and mobility. He relies on a wheelchair and is hoist transferred, he has all his nutrition, hydration and medication via gastrostomy tube (PEG and Jej). He relies on others to keep him safe and meet all his daily needs.
Adam lives in his family home with his father, his siblings, and a pet dog. He has a team of support workers, and we are looking for an experienced individual to provide leadership and day to day management to the team, as well as working with Adam alongside his other staff.
Duties Will Include:
The main role of the Team Leader is to manage a team of support workers and ensure they always deliver excellent care and assistance to Adam, working closely with the family, the Case Manager and members of the MDT to achieve this.
- To provide full assistance when on shift to meet Adam’s care requirements, including with feeding, mobility, medication, mouth care, dressing, toileting, changing, showering and health monitoring, ensuring high standards of care as per the care plan
- To monitor Adam’s health, escalating any concerns to the appropriate people, alerting his parents and the Case Manager to any changes
- To assist and support Adam in his home and wider community, including with his mobility and driving him to activities and events
- To contribute to the planning of activities and holidays, making necessary arrangements with the approval of Adam’s parents and Case Manager. To empower him within any given environment
- To keep detailed and accurate records, reporting any changes to the family or Case Manager in a timely manner, ensuring other staff do the same
- To be the main point of contact between the team and the Case Manager; working with Ben Holden Ltd to manage and support staff induction, training and dealing with disciplinary issues as they arise
- To be individually accountable for standards of professional practice and to undertake all other related duties commensurate with the general level of responsibility of the post
- Provide regular supervision for all employees and complete annual appraisals as well as be available to provide some out of hours support
- To manage the rota, annual leave and sickness, ensuring there is adequate staffing cover
- To assist the Case Manager with writing and updating care plans and associated risk assessments
